We present the case of a patient from the Atlantic zone, initially diagnosed with hypertensive and ischaemic cardiopathy, referred for a pacemaker implant due to a trifascicular block (atrioventricular block, right bundle branch block and left anterior bundle branch block). After the implant, a positive serology for Chagas disease was reported. This was the only case, out of a total of 241 patients that received implants of cardiac devices and were screened for this disease during a period of 22 months; the majority of these patients came from some zones of the province of San José and from the provinces of Cartago and Limón. This low prevalence has already been reported in the country and contrasts with the high percentage in other countries where the disease is endemic.
