Open-access Program of pharmaceutical attention and pursuit farmacology

The following are the results of surgery program and pharmaceutical follow-up done in the Hospital William Allen de Turrialba (HWA) during the months of February to May, 2004. A total of 46 patients talked about the pharmacy service, 68% of these were older than 50 years, with an average of 1,74 medical problems, in its majority hypertense (25%). The mode of medicine consumption was of 6 and an average of 7,23 ± 3.84 medicines by patient. 81,5% of the preoccupations of the patients had some relation with medicines. There found were 34 problems of the patient, 3 problems of storage and 34 problems related to the treatment. In 122 pharmaceutical consultations a total of 156 activities were done. 95% of the patients referred that the initial problems have been solved; the same percentage considers that they have acquired enough confidence in the use of their treatments, 85% referred that they use them correctly; now the treating doctors consider that their patients use medicines better and the problems of these have been solved moderately, that it is a must to continue with the pharmaceutical follow-up, and even extend it to the patients` relatives and home flying visit.
